How to become a Lieber Fine Wines & Spirits Customer

New York

  • A valid license issued from the New York State Liquor Authority.
  • A completed ST-120 form and a copy of the Certificate of Authority.
NY Application

New Jersey

  • A valid license issued from the New Jersey State Liquor Authority.
  • A completed ST-3 form and a copy of the Certificate of Authority.
NJ Application
In order for Lieber to extend credit. If you are the owner/officer of the establishment that holds the liquor license, please complete and sign all the forms.
